• PC (Windows) users, if you don't already have Adobe Acrobat (Standard or Pro.) Download Adobe Acrobat Standard here FREE.
  
  Mac users, you can download Download Adobe Acrobat Standard here FREE, but do not need it if you are running MAC OSX 10.4 or later. Preview will allow you to open, edit and save Adobe PDF Documents.
  
  Computer Illiterate People can just print the document, fill it out and submit it
